We will request your OHIP number for laboratory and diagnostic tests and referral to specialists if required. Please note we do not provide prescriptions for narcotics or controlled substances. Fees are non-refundable with less than 4 hours notice and for no shows.
Additional fees may be required for form completion, referral to specialist, follow-ups to discuss diagnostic test results.
